We’ve tiled the wall off the bathroom behind our vanity and toilet with decorative tile. Initially I decided against a medicine cabinet, but now I’m worried that I won’t be able to install one later if I change my mind.

Yes but it will be more difficult since we would have to neatly score the tile and determine how far back into the wall you would like your medicine cabinet to go. Cutting the tile will be more time consuming, not to mention the risk of breaking.
It would be complicated, but a tile grinder wheel will cut the hole for the cabinet. If the tile is installed properly, it shouldn’t break the tile. You just need to work slow and with a sponge to keep it wet.
