Depends on the thickness of your vinyl and manufacturer. A moisture barrier should be placed on top of the concrete as well. If your vinyl is sturdy and thick and holes are that small, and you have a moisture barrier, I wouldn’t say it’s necessary, especially if what you are referring to are the holes caused from carpet tack strip being removed from concrete. By the time you factor in new baseboards and quarter round you’re nearly covering those holes with it anyways. Now if you have long cracks running across middle of floor you can either grind them flat if they are peaking, or if they are sunken you can use plain patch or a quick drying concrete like quik crete. Find these at your local home depot or lowes.
In addition to the “yes, you want the best level surface”, you also want to be sure to first thoroughly clean the cement and use a primer before applying the self leveler. Each self leveler will state how large of a void it will fill, you may need to use a patch filler before the leveler, and again it’s important to clean and prime first or else the patch can separate and crack up over time and use.
Yes, you will need to level the floor recesses so you do not get mirroring. Even when using a waterproof or similar underlayment, you want to make sure the floor is as level as possible. Especially with vinyl plank flooring.
